DOMUS Primary Care (“DOMUS,” “we,” “us,” or “our”) respects your privacy. This page has three parts. Part I explains what information this website, domus.doctor, collects and how it is used. Part II is our Notice of Privacy Practices required by HIPAA — it describes how medical information about you may be used and disclosed, and how you can get access to it. Part III covers text messaging: what we collect when you agree to receive text messages, how that consent is obtained, who handles the messages, and how to stop.

Part I — Website Privacy


What this website collects

This website does not require an account or login. The information handled by the site is limited to:

Analytics and advertising measurement

We use Google Analytics to understand, in aggregate, how visitors find and use this website, and Google Ads conversion measurement to learn whether our advertising works. These tools use cookies and collect information such as the pages you view, the approximate area you are visiting from, your device and browser type, and how you arrived at the site.

We have configured these tools narrowly. Google Signals and all advertising-personalization features are turned off; we do not build advertising profiles and we do not use remarketing. We do not upload contact lists or any patient information to Google. Detailed analytics records are automatically deleted after 14 months. These tools run only on this marketing website, domus.doctor. Our booking and patient-portal site, operated by IntakeQ, carries no analytics or advertising code of any kind, and nothing collected on this website is connected to your medical records or your identity as a patient.

If you reach this site from one of our ads and call us, the phone number shown to you may be a Google forwarding number. Google connects your call directly to the practice and reports the call’s date, time, duration, and area code so we can tell that our advertising led to a call. Calls are not recorded or transcribed, and the reports we receive do not identify you.

You can opt out of Google Analytics with the browser add-on at tools.google.com/dlpage/gaoptout, or by blocking cookies in your browser settings. The site works fully without these cookies.

Cookies and local storage

Google Analytics and Google Ads set first-party cookies (names beginning with “_ga” or “_gcl”) used to distinguish browsers and measure visits and ad conversions. The site also saves a single value (“lang”) in your browser’s local storage to remember whether you prefer English or Russian. You can remove any of these at any time by clearing your browser’s site data; the site works fully without them.

Booking and the patient portal

Online booking and the patient portal are provided by IntakeQ, a practice-management platform built for HIPAA-covered medical practices. When you book a visit or complete intake forms, you do so on IntakeQ’s secure site at domus.intakeq.com. Health information you provide there is protected health information and is handled as described in Part II below.

Contacting us

If you email contact@domus.doctor or call or text (332) 373-1411, the information you share comes directly to the practice and is used only to respond to you. Please do not send detailed medical information by regular email — for clinical questions, the patient portal is the secure channel. If you text us, see Part III for how text messaging is handled.

Children

This website is not directed at children under 13, and we do not knowingly collect personal information from children through it. Care for patients under 18 is arranged by a parent or guardian.

Changes to this page

If our website practices change, we will update this page and the “Last updated” date above.

Part II — Notice of Privacy Practices


Effective date: October 1, 2025

Your information. Your rights. Our responsibilities. This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Your Rights

When it comes to your health information, you have certain rights. This section explains your rights and some of our responsibilities to help you.

Get an electronic or paper copy of your medical record

  • You can ask to see or get an electronic or paper copy of your medical record and other health information we have about you. Ask us how to do this.
  • We will provide a copy or a summary of your health information, usually within 30 days of your request. We may charge a reasonable, cost-based fee.

Ask us to correct your medical record

  • You can ask us to correct health information about you that you think is incorrect or incomplete. Ask us how to do this.
  • We may say “no” to your request, but we’ll tell you why in writing within 60 days.

Request confidential communications

  • You can ask us to contact you in a specific way (for example, home or office phone) or to send mail to a different address.
  • We will say “yes” to all reasonable requests.

Ask us to limit what we use or share

  • You can ask us not to use or share certain health information for treatment, payment, or our operations. We are not required to agree to your request, and we may say “no” if it would affect your care.
  • If you pay for a service or health care item out-of-pocket in full, you can ask us not to share that information for the purpose of payment or our operations with your health insurer. We will say “yes” unless a law requires us to share that information.

Get a list of those with whom we’ve shared information

  • You can ask for a list (accounting) of the times we’ve shared your health information for six years prior to the date you ask, who we shared it with, and why.
  • We will include all the disclosures except for those about treatment, payment, and health care operations, and certain other disclosures (such as any you asked us to make). We’ll provide one accounting a year for free but will charge a reasonable, cost-based fee if you ask for another one within 12 months.

Get a copy of this privacy notice

  • You can ask for a paper copy of this notice at any time, even if you have agreed to receive the notice electronically. We will provide you with a paper copy promptly.

Choose someone to act for you

  • If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your health information.
  • We will make sure the person has this authority and can act for you before we take any action.

File a complaint if you feel your rights are violated

  • You can complain if you feel we have violated your rights by contacting us using the information at the bottom of this page.
  • You can file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights by sending a letter to 200 Independence Avenue, S.W., Washington, D.C. 20201, calling 1-877-696-6775, or visiting www.hhs.gov/ocr/privacy/hipaa/complaints/.
  • We will not retaliate against you for filing a complaint.

Your Choices

For certain health information, you can tell us your choices about what we share. If you have a clear preference for how we share your information in the situations described below, talk to us. Tell us what you want us to do, and we will follow your instructions.

In these cases, you have both the right and choice to tell us to:

  • Share information with your family, close friends, or others involved in your care
  • Share information in a disaster relief situation
  • Include your information in a hospital directory

If you are not able to tell us your preference, for example if you are unconscious, we may go ahead and share your information if we believe it is in your best interest. We may also share your information when needed to lessen a serious and imminent threat to health or safety.

In these cases we never share your information unless you give us written permission:

  • Marketing purposes
  • Sale of your information
  • Most sharing of psychotherapy notes

In the case of fundraising:

  • We may contact you for fundraising efforts, but you can tell us not to contact you again.

Our Uses and Disclosures

How do we typically use or share your health information? We typically use or share your health information in the following ways.

Treat you

We can use your health information and share it with other professionals who are treating you.

Example: A doctor treating you for an injury asks another doctor about your overall health condition.

Run our organization

We can use and share your health information to run our practice, improve your care, and contact you when necessary.

Example: We use health information about you to manage your treatment and services.

How else can we use or share your health information? We are allowed or required to share your information in other ways — usually in ways that contribute to the public good, such as public health and research. Help with public health and safety issues

We can share health information about you for certain situations such as:

  • Preventing disease
  • Helping with product recalls
  • Reporting adverse reactions to medications
  • Reporting suspected abuse, neglect, or domestic violence
  • Preventing or reducing a serious threat to anyone’s health or safety

Do research

We can use or share your information for health research.

Comply with the law

We will share information about you if state or federal laws require it, including with the Department of Health and Human Services if it wants to see that we’re complying with federal privacy law.

Respond to organ and tissue donation requests

We can share health information about you with organ procurement organizations.

Work with a medical examiner or funeral director

We can share health information with a coroner, medical examiner, or funeral director when an individual dies.

Address workers’ compensation, law enforcement, and other government requests

We can use or share health information about you:

  • For workers’ compensation claims
  • For law enforcement purposes or with a law enforcement official
  • With health oversight agencies for activities authorized by law
  • For special government functions such as military, national security, and presidential protective services

Respond to lawsuits and legal actions

We can share health information about you in response to a court or administrative order, or in response to a subpoena.

Special Provisions Under New York Law

HIV-related information

We will not disclose HIV-related information without your written consent, except when required or permitted by law.

Mental health records

Mental health treatment records have extra protection under New York law. We will not share these records without your authorization, unless disclosure is required by law.

Substance use disorder treatment

Substance use treatment information is protected by federal and New York law. We cannot release these records without your written consent, except in limited circumstances allowed by law.

Genetic testing information

Genetic test results are confidential under New York law. We will only share this information with your explicit consent or when required by law.

Minors and confidential services

In certain cases, minors may consent to their own care (such as reproductive health, STI treatment, or mental health services). These records may be kept confidential from parents or guardians as required by law.

Patient access and record retention

We maintain medical records for at least six years, and longer for minors as required by law. You have the right to access or obtain copies of your records in accordance with HIPAA and New York law.

Breach notification

If your information is compromised, we will notify you as required by HIPAA and New York’s Information Security Breach and Notification Act.

Changes to the Terms of This Notice

We can change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available upon request, in our office, and on our website.

Part III: Text Messaging (SMS)

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties.

This part describes the text messages DOMUS Primary Care sends and receives at (332) 373-1411, and what we do with the information involved. The terms of the messaging program itself are on our Terms of Service page.

What we collect for text messaging

How we obtain your consent

There is no text message consent checkbox on this website. Consent is obtained in one of three ways:

Consent to receive text messages is not a condition of receiving care.

How we use it

We use your mobile number to send appointment logistics, care communication, and billing and administrative messages, and to reply when you text us. We do not send marketing or promotional text messages. If that ever changes, we will ask for your consent separately before sending any.

Who handles these messages

Text messages to and from (332) 373-1411 are carried by RingRx, our telephone and messaging vendor, which operates under a signed Business Associate Agreement with DOMUS Primary Care and is bound by HIPAA. Scheduling and intake messages may also involve IntakeQ, our practice-management platform, on the same footing. Your own mobile carrier handles the message in transit and is not covered by either agreement, which is why standard text messaging is not a secure channel. We keep health details in text messages to the minimum necessary.

Sharing

We do not sell your mobile number. We do not share it with third parties or affiliates for marketing or promotional purposes, and we do not share it with lead generators, data brokers, or advertising networks. Your consent to receive text messages is never shared, rented, sold, or transferred to anyone. The only parties that handle your mobile number are RingRx, IntakeQ, and your own mobile carrier, each of them solely to deliver the messages you asked for.

Health information contained in a message is protected health information and is handled under Part II above.

How long we keep it

Your mobile number and the record of your consent are kept while you are a patient of the practice, and afterward for the six-year period that applies to medical records under New York law. If you opt out, we keep a record of the opt-out itself so that we do not text you again.

How to opt out

Reply STOP to any message from us and we will stop sending them. You will receive one final message confirming it. Reply HELP to any message, or call or text (332) 373-1411, for assistance. Message and data rates may apply. Opting out of text messages does not affect your care or your membership; we can reach you by phone, email, or the patient portal instead.
